Introduction

Most administration errors described in the literature are unit errors: a concentration confused with a dose, a syringe scale read as milligrams, or a dead-space loss not accounted for. This portal collects the practical articles and the calculator that makes the arithmetic explicit.

These articles describe what labels and pharmacopoeias state. Nothing here is a recommendation, and nothing here is medical advice.
